A significant measles outbreak in the Southwest United States, primarily centered in Texas and New Mexico, has resulted in nearly 100 confirmed cases. Health officials suspect the actual number of infections may be much higher—potentially between 200 and 300—due to unreported or undiagnosed cases. The affected region includes a close-knit Mennonite community in Gaines County, known for lower vaccination rates, which has likely contributed to the outbreak’s scale. Vaccination exemptions among children in this county have risen sharply over the past decade, with over 17.5% of kindergarteners exempted in 2023, compared to 7.5% in 2013, and some local schools reporting exemption rates nearing 48%.
In Texas, the outbreak is concentrated in the South Plains region, with Gaines County identified as the epicenter. By February 21, the Texas Department of State Health Services reported 90 confirmed cases since late January, with 77 of these occurring in children and teens under 17. Of these, 85 cases involved individuals who were either unvaccinated or had an unknown vaccination status, and 16 people have been hospitalized. The outbreak has also spread to nearby counties, including Terry (nine cases), Yoakum (two cases), Lynn (one case), and Lubbock (one case).
In neighboring New Mexico, nine cases have been confirmed in Lea County, which borders Gaines County, as of February 22. These cases include an initial unvaccinated teenager with no known travel history or link to the Texas outbreak, followed by two additional unrelated cases by mid-February, with the total rising to nine by the latest reports. The spread in New Mexico underscores the disease’s ability to cross state lines, especially in areas with interconnected populations.
Measles is a highly contagious viral disease, spread through respiratory droplets from coughing, sneezing, or breathing. It can lead to serious complications, particularly in young children, including pneumonia, encephalitis (brain swelling), and, in rare cases, death. The measles, mumps, and rubella (MMR) vaccine, given in two doses, is 97% effective at preventing infection, with the first dose recommended at 12–15 months and the second between ages 4 and 6. However, vaccination rates in the U.S. have slipped below the 95% threshold needed for herd immunity in recent years, with only 93.1% of kindergarteners vaccinated in the 2022–23 school year, leaving approximately 250,000 children vulnerable nationwide. In the Southwest outbreak, nearly all cases (95% according to the CDC) have occurred in individuals who were unvaccinated or had an unknown vaccination status, with only 4% having received one dose and none fully vaccinated with two doses.
The U.S. Centers for Disease Control and Prevention (CDC) reported a national total of 93 cases across eight states by February 20, with the Southwest outbreak driving a dramatic increase from just 14 cases the previous week. This five-fold jump prompted the CDC to highlight three distinct outbreaks in 2025, with 92% of cases linked to these clusters.
